Grazing muzzle

My horse wears a grazing muzzle but manages to get it off every time.  It is adjusted correctly, but he still manages to figure out how to get it off. Any suggestions would be appriciated.

Stops dead and eats grass

My horse, either at a walk or trot, will stop IMMEDIATELY, without warning, and eat grass. He is fed properly and grazes in pasture.
